But what exactly is a compounding pharmacy? And what makes some compounding pharmacies, known as 503B outsourcing facilities, so critical to modern healthcare?

A Brief Look Back

Compounding pharmacies have been around for centuries—long before mass-produced medications were the norm. In the earliest days of medicine, pharmacists prepared customized remedies by hand, adjusting dosages, ingredients, and formulations to suit individual patients. (Amy Leitman, JD, President of NTMir Contributor, USA Today) - For most Americans, the letters "NTM" don’t mean anything. But for tens of thousands of people across the country, they represent something serious: a chronic lung disease that’s often overlooked, misunderstood, and on the rise.

NTM, or nontuberculous mycobacterial lung disease, isn’t contagious. But that doesn’t make it harmless. Caused by bacteria found in soil and water, NTM can lead to progressive lung damage, especially in people with underlying respiratory conditions. What’s most alarming is how easily it hides in plain sight—mimicking other illnesses, evading detection, and often going untreated for years.

As we approach World NTM Day on August 4, it’s time to bring this disease out of the shadows.

NTM lung disease isn’t rare anymore. Over the past few decades, researchers have tracked a steady increase in infections across the U.S., especially among people over 65 and those with structural lung issues like bronchiectasis or COPD. According to the Centers for Disease Control and Prevention, cases are growing by 8% annually. Currently there are estimated to be more than 200,000 cases in the U.S. alone.

Certain regions are being hit harder than others. States like Florida, Hawaii, California, and Texas report disproportionately high rates, with urban hubs like Chicago, Atlanta, Los Angeles, and New York City also seeing spikes. The combination of humid climates, aging populations, and exposure to aerosolized water from showers, hot tubs, or soil seems to provide the perfect conditions for the bacteria to thrive.

Still, it’s not limited to one geographic area. Wherever people are inhaling air from contaminated environments—and especially where structural lung conditions are common—NTM is a growing risk.

One of the most frustrating aspects of NTM is how easy it is to mistake it for something else. People often develop a persistent cough, fatigue, night sweats, or shortness of breath—symptoms that could point to any number of respiratory conditions. It’s not unusual for patients to be told they have bronchitis or asthma, and to be treated accordingly.

But when weeks turn into months, and standard treatments don’t work, it’s time to look deeper.

Diagnosis requires a chest CT scan and sputum cultures, but even then, identifying the right strain and choosing the right treatment takes time. The standard course involves multiple antibiotics taken for over a year, and in many cases, patients can be reinfected with another strain later on.

Behind the data are stories—some hopeful, some heartbreaking.

Diane, a retired teacher in Houston, spent more than two years bouncing between physicians. “I had a chronic cough I just couldn’t shake,” she says. “They thought it was allergies or maybe long COVID.” After finally seeing a pulmonologist who tested for NTM, she got her diagnosis. “I was relieved, but scared. I had never even heard of it.”

In Miami, Michael, 67, had a similar experience. “They told me it was COPD, and I believed them. But my energy kept getting worse.” It wasn’t until a specialist ordered a specific culture that the real cause came to light. “By then, my lungs were already in bad shape.”

These aren’t rare anecdotes—they’re reflective of a larger issue: too many patients are slipping through the cracks because NTM isn’t top of mind for most clinicians.

Every year, August 4 marks World NTM Day, an international effort to raise awareness about this under-recognized illness. The day offers an opportunity to elevate patient voices, educate health professionals, and encourage policy solutions to improve care.

Jeff never expected a persistent cough to turn into something serious. But in early 2021, after a scan revealed advanced-stage lung cancer, his life changed in an instant. The cancer had spread to his bones, ribs, clavicle, and brain, initially considered inoperable.

Jeff refused to give up. He educated himself and pursued biomarker testing. While every cancer journey is different, research made more options available to him, and that made a difference.

“I want my story to focus on the disease and the cure. It is because others before me advocated and made their needs heard that biomarker testing was discovered and, in turn, new highly effective drugs were developed. Getting more involved in research and doing my own research has become my purpose.”

Research That Breathes Life Into the Future

Behind stories like Jeff’s are the scientists pushing boundaries in lung cancer treatment. Dr. Zhen Zeng of Johns Hopkins University School of Medicine was recently awarded a prestigious Early Career Researcher Grant of $200,000 through LCFA.

Dr. Zeng’s work uses cutting-edge bioinformatics to identify tumor-reactive T cells, a breakthrough that could lead to more personalized and effective immunotherapy for patients.

“This grant gives me the resources to move faster,” says Dr. Zeng. “And that means earlier detection, better treatment options, and more lives saved.”

The impact of early detection is staggering: catching lung cancer at Stage I could improve five-year survival rates by more than 50%.

Why does this happen? The summer heat often means more indoor get-togethers and events in air-conditioned spaces, more travel, and more time spent around other people. These are just a few of the things that give the virus more chances to spread.

Oral health is closely tied to not just our physical wellness but also our mental and financial well-being.

Unfortunately, far too many older Americans aren’t getting the dental care they need. In fact, one in four U.S. adults aged 65 and older have not had dental care in more than two years. With some Medicare plans scaling back dental benefits, now is the time to act.

The well-being connection

Oral health can have a lasting impact on your broader physical wellness. Without proper care, harmful bacteria can build up around teeth and cause gum disease or even spread to other parts of the body. Dry mouth, which can be caused by some medications as well as by CPAP use, can increase the risk of cavities and gum disease as well. (NewsUSA) - A once-a-day pill called blarcamesine may slow memory loss and preserve independence—without the scary side effects or burdens of current treatments.

If you love someone with Alzheimer’s, you know how quietly devastating the disease can be. One day it’s a forgotten name. A year later, it’s forgotten faces, forgotten routines. For many, even the latest treatments feel out of reach: risky, invasive, or simply too late.

But now, a new study offers something rare in Alzheimer's, a reason for hope. A once-daily pill called blarcamesine (pronounced blar-CAM-uh-seen) may actually slow the disease’s grip. And it does so safely, without the brain swelling and bleeding linked to some newer drugs.

The data, presented this spring at an international Alzheimer’s conference, showed that people who started taking blarcamesine early and stayed on it continuously had better memory, clearer thinking, and greater ability to handle everyday tasks—even after nearly four years.

For families trying to hang on to time and dignity, that’s no small thing.

As Dr. Timo Grimmer, a lead scientist on the study, put it: “Diagnosing and treating people earlier and consistently may lead to greater clinical benefit.”

Blarcamesine isn’t an infusion. There’s no monthly IV. No MRI scans. No trips to specialty hospitals. Just one pill a day.

That simplicity matters. Many Alzheimer’s treatments today require hospital visits, specialized imaging, and high-risk monitoring for side effects. These hurdles often make access harder for families without nearby clinics or flexible jobs.

"Oral medications like blarcamesine are more convenient for many patients as they can take medication at home without requiring regular clinic visits or infusion appointments," says Amy Reichelt, Ph.D., a neuroscientist and drug development expert. "Monoclonal antibodies require regular infusion center visits (typically every 2–4 weeks), which creates significant logistical burdens for patients and caregivers."

Still, there are trade-offs. “Adherence challenges exist with oral Alzheimer's medications as patients can forget to take their medications,” Reichelt adds. “However, the convenience of home administration is valuable for Alzheimer's patients who may struggle with transportation and clinic visits as their condition progresses.”

Let’s put the science in plain terms.

In the study, people who began taking blarcamesine right away had stronger thinking skills than those who started a year later. Their memory declined more slowly, and they were better able to handle basic tasks like dressing or preparing food.

"A recent Phase 2b/3 clinical study with blarcamesine showed that there was no evidence of neurological tissue damage such as hemorrhage or Amyloid-related imaging abnormalities (ARIA), which have been observed with lecanemab and donanemab," Reichelt explains. Those antibody drugs require regular MRI scans to monitor ARIA, a side effect that can force treatment to stop.

"The risk of ARIA requires frequent patient monitoring by MRI and can lead to treatment discontinuation," she says. "However, the frequent monitoring with MRI might catch increasing disease progression or other pathologies."

While safety and simplicity are headline features, the economics of a pill like blarcamesine could be just as powerful.

“Key economic factors include reduced monitoring costs, as frequent MRI scans are costly,” Reichelt notes. “And some individuals are incompatible with MRIs due to metal implants or claustrophobia.”

IV infusions come with additional costs—nursing time, facility fees, medication prep, and more. “Moreover, caregiver time, transportation, and lost productivity for caregivers dealing with regular infusion appointments can be substantial, particularly as patients become less mobile,” Reichelt adds.

“Oral medications typically have broader insurance coverage and accessibility, especially in rural or underserved areas where access to infusion centers may be limited,” she says. “The potential cost savings of an oral medication without MRI monitoring is likely significant for both insurance payers, and patient experience and treatment adherence may also be improved.”

Blarcamesine is still under review by health authorities in Europe. It’s not yet approved for general use in the U.S. But the signs are encouraging, and the pressure is growing.
